About the Role:

The Social Care Navigation team forms the backbone of Papa, playing a crucial role in enhancing our members' health outcomes by identifying and mitigating barriers to care. Our team steps in when Papa's Pals and Care Team members unearth member needs or health risk factors during their in-person visits and telephone interactions. These insights inform our Social Care Navigators, who reach out to members, providing them with pertinent resources. This proactive approach helps us build robust relationships with members, facilitating the detection of further care gaps.

Our Navigators are dedicated to comprehending the obstacles to care and furnish resources to alleviate the identified challenges. They guide members through their health plan benefits and synergize effectively with healthcare providers, the Care Team, Papa pals, and Community-based Organizations. Their collaborative efforts aim to ensure positive health outcomes for our members. This role will work alongside a small, passionate team of other Social Care Navigators and report directly to the Senior Clinical Operations Manager.

What you'll be doing:

  • Outreach Papa’s members by phone to validate needs identified, understand underlying barriers and take action to address them
  • Communicate with members’ Care Teams (Care Coordinators, primary care physicians and other health care providers) to facilitate care
  • Identify resources, including in-person Pal visit, that can assist members in overcoming care barriers like transportation, housing, food insecurity, and medication adherence.
  • Follow-up with our members, to ensure their health needs are effectively met, and any obstacles they may face are promptly addressed
  • Maintain a comprehensive knowledge of our partners’ health plan benefits including mental health, over-the-counter medicine allowances, and medical coverage, to ensure members understand and use all resources available to them
  • Utilize internal applications to thoroughly document your interactions with members for reference

More about you:

  • Ideally 1-2 years of experience coming from a health provider office or insurance setting
  • Ability to build relationships with different types of people, including members, organization members, and health care providers
  • Passionate, trustworthy, and empathetic when working with members
  • Flexible and adaptable in response to changing client and health care providers’ needs
  • Professional and clear communication and interpersonal skills when interacting with members, clients, healthcare professionals, and internal teams
  • Understanding of medical terminology
  • Bilingual (English/Spanish) preferred

Job Type: Full-time, Non-Exempt

Hours: 11am-8pm ET Tuesday-Saturday
